There is no better way to fall or fail than using wrong data. A thorough research and studies shuld be done in order to achieve a great anchor to keep the business up and running.

A lot of these have something to do with how leaders or people running the company are equipped with the knowledge and skills to make decisions. There will be biases along the way but it will all come up to identifying and making up for the bad decisions.
