In 2014, Formé opened a brand new 11,000 square foot medical facility in White Plains, New York, doing business as Formé Urgent Care and Wellness Center. This state-of-the-art facility operates as a full service Diagnostic and Treatment Center that provides the following services; Walk-In Medical Care, Physical and Occupational Therapy, Radiology, Diagnostics and Outpatient Surgery. Formé is opened 365 days a year and is overseen by board certified emergency medicine physicians. Formé Medical Directors, physicians and practitioners are all board certified.
Coming spring 2016, Formé will be adding multi-specialty physicians to the center, allowing the residents of Westchester to have access to compassionate and dignified specialized care all under one roof.

We are open 7 days a week, 365 days a year.
Monday-Saturday: 8am-9pm
